real time exits on monthly bars

  Is there any way of triggering an exit or entry on the one month timeframe in real time rather than at the close or open?

  Yes, add a smaller timeframe as  DEFAULT (the one on the chart), but use the Monthly TF (with or without UpdateOnClose)  for your SETUP.

  If you search the forum for the word MTF  or MULTI TIME FRAME, you will be returned plenty of link to articles, posts and lots of code snippets about MTF support.
